HACC – Home and Community Care Program Transition

The WA Home and Community Care (HACC) and its successor Continuity of Support (COS) Program are closed programs that no longer accept new clients.

From 1 July 2021 the majority of HACC clients transferred from HACC to COS. Only a small number of clients remain with HACC service providers.

The Purchasing and Contracting Unit (PCU) is working closely with a small number of HACC service providers to support these clients to transition to COS providers (or to National Disability Insurance Scheme (NDIS) Commonwealth Home Support Programme (CHSP) as appropriate) over the next 6 - 12 months.

Existing levels of support to eligible clients will be maintained to the maximise their independence, well-being and connections to the community.

Commonwealth Home Support Programme (CHSP)

As of 1 July 2018 services for older people (people aged 65 and over and Aboriginal and/or Torres Strait Islander people aged 50 and over) were transitioned and are now funded and managed by the Commonwealth Department of Health through the CHSP.

The CHSP provides service types similar to those provided by the HACC/COS Program. These services include help with housework, personal care, meals, social support, nursing care, equipment, allied health and home maintenance.

National Disability Insurance Scheme (NDIS)

From July 2017, services for younger people (people aged under 65 and Aboriginal and/or Torres Strait Islander people aged under 50) have been transitioning out of the WA HACC/COS Program to the NDIS as it rolls out across Western Australia.

As of 1 July 2019, the NDIS entered its final rollout phase in Western Australia. This means that younger people living in all regions of Western Australia are now able to test their eligibility for the NDIS.

With the NDIS now accepting younger people from all regions, the WA HACC/COS Program is only available to younger clients who are already recipients of WA HACC/COS services. To ensure continuity of support, younger people who were receiving HACC services as at 30 June 2019 will continue to receive services while they test their eligibility for the NDIS.
