What is an Emergency Department Care Coordination Team?

Emergency Department Care Coordination Teams (ED CCTs) are located in each public hospital emergency department in Perth. These multidisciplinary teams support the busy emergency departments, providing primary functions of patient risk screening, assessing and directing patients to appropriate care and support.

A large part of the work entails care planning to assist patients back to home when returning home is safe. The ED CCT may make referrals to community support services and to relevant specialist services as required.

Most patients seen by an Emergency Department Care Coordination Teams are older. Younger patients are seen as required.

Cost

This service is provided as part of public hospital care. There is no cost to the patient.
