This communications toolkit aims to promote the Manage My Care app. It is a free mobile and web-based app that provides consumers with an efficient way to track public outpatient appointments and referrals for themselves or those in their care.

- Increase registered users of Manage My Care.

Manage My Care provides patients and their carers:
- 24/7 access to real-time outpatient appointment and referral information
- an easier way to update contact details for yourself or those in your care.
Target audience
- People in Western Australia 16+ who have outpatient appointments and referrals or who manage these for someone in their care.
